Application Form (Official)
Use the official embassy form provided by Havenus. Both signature fields must match your passport signature exactly — mismatches will be rejected by the embassy. Word (.docx) or PDF accepted.

Passport (Actual)
Original passport submitted to embassy. Must be valid for 6+ months from travel date.

ARC Copy (Front)
Front side of your ARC. If expired, substitute with Certificate of Alien Registration (외국인등록사실증명원).

ARC Copy (Back)
Back side of your ARC.

Photo — Print (3.5×4.5cm)
Passport-style print, taken within last 6 months. White background.

E-Ticket
Flight booking confirmation (round-trip or departure).

Hotel Voucher
Your English name (exactly as in your passport) must appear on the booking confirmation.

Schedule of Stay (Official)
Use the official embassy form provided by Havenus. Fill in your day-by-day travel plan. Word (.docx) or PDF accepted.

Bank Balance Certificate (English)
Must be issued within 1 week of embassy submission date. English version required.

Employment Certificate
Issued by your current employer in Korea. Students: submit Enrollment Certificate (재학증명서) instead.

Income Certificate (소득금액증명원)
Must show ₩40,000,000+ annual income. Issue from Hometax (홈택스), based on previous year income. Must be issued within 2 weeks. 갑근세원천징수영수증 is NOT accepted.

Multiple Entry Statement (Official)
Use the official embassy form (복수비자발급신청사유서) provided by Havenus. Must be handwritten. Signature must match your passport signature exactly.
